Team Check-in Information

Check-in information is provided with your team's acceptance letter.

You MUST check-in at one of the dates/times provided. Check-in at the tournament site is only available to teams with a waiver. Waivers are available for out of state and out of region teams (out of region includes Castle Rock (and further south) and Loveland (and further north)). If you do not check-in prior to the tournament, a $50 charge will be assessed to your team and must be paid before your first game is played.

No roster changes are allowed after check-in.

What to Bring to Check-in

The following must be provided at the time of check-in:

  • Two copies of your team's non-laminated, signed, and state certified Fall 2015 roster

  • Two copies of completed Guest Player Roster (download Guest Player Form)
  • Player passes for all players (including guest players) for Fall 2015
  • Passes for each coach for Fall 2015

  • Copies of all players’ SIGNED medical release forms (download Medical Release Form - English/Medical Release Form - Spanish)
  • If traveling from outside of Region 4 and playing under USYS: Two copies of your state’s Permission to Travel Papers
