Team Check-in Information

Check-in information is provided with your team's acceptance letter.

You MUST check-in at one of the dates/times provided. Check-in at the tournament site is only available to teams with a waiver. Waivers are available for out of state and out of region teams (out of region includes Colorado Springs, Ft. Collins and Greeley). If you do not check-in prior to the tournament, a $50 charge will be assessed to your team and must be paid before your first game is played.

No roster changes are allowed after check-in.

What to Bring to Check-in

The following must be provided at the time of check-in:

  • Two copies of your team's non-laminated, signed, and state certified Fall 2014 roster

  • Two copies of completed Guest Player Roster (download Guest Player Form)
  • Player passes for all players (including guest players) for Fall 2014
  • Passes for each coach for Fall 2014

  • Copies of all players’ SIGNED medical release forms (download Medical Release Form)
  • If traveling from outside of Region 4 and playing under USYS: Two copies of your state’s Permission to Travel Papers
