Check-In Requirements

Please note the following check-in requirements. If you have any problems or questions, contact Karen Logan at shootout@trebolsoccer.org.

You are required to provide the following at the time of Check-in:

• Two copies of your state certified Fall 2012 roster
• Player passes for all players, including guests – must be for Fall 2012/Spring 2013
• Passes for each Coach
• Two copies of Completed Guest Player Roster (Download Guest Player Form)
• If you are from outside of Colorado and playing under USYS - Two copies of your State’s Permission to Travel Papers
• Copies of all players’ medical releases (Download Medical Release Form)
• Please note, no roster changes are allowed after check-in

Your team is required to "check-in" in advance. 

Failure to check-in prior to Friday, October 5th will incur a $50 convenience fee.

 Convenience Fee Waivers are available from the Tournament Director for out-of-state and     out-of-region teams (in-region is the Front Range I-25 corridor).
