Liability Release and Authorization for Medical Treatment:
My child, has my permission to play soccer with Alfred J. Hurley and First Touch Soccer Academy.
On my child’s behalf, I hereby release Alfred J. Hurley and all other persons affiliated with First Touch Soccer Academy of liability for injury from risks normally associated with playing or watching soccer. I additionally release the City of Boulder, Boulder County, and the owners of all fields from any and all liability associated with my child’s participation in First Touch Soccer Academy and Alfred J. Hurley.
I authorize Alfred J. Hurley, the coaches, assistants or training officials to obtain medical attention for my child in case of emergency if unable to reach the physician stated above and I release them from any responsibility for such medical attention

What to Bring and Wear:
All participants are expected to arrive in soccer attire: soccer/athletic shorts, tee-shirt, soccer shoes/cleats, and shin guards (required). No jewelry, please.
