Check-In Requirements

Please note the following check-in requirements. If you have any problems or questions, contact Karen Logan at [email protected].

You are required to provide the following at the time of Check-In:

• Two copies of your state certified Fall 2011 roster
• Player passes for all players, including guests – must be for Fall 2011/Spring 2012
• Passes for each Coach
• Two copies of Completed Guest Player Roster (Download Guest Player Form)
• If you are from outside of Colorado and playing under USYS - Two copies of your State’s Permission to Travel Papers
• Copies of all players’ medical releases (Download Medical Release Form)
• Please note, no roster changes are allowed after check-in

There are two opportunities to Check-in for the Colorado Shootout.  You may attend either but you must attend one of them.
